This is Sincerity Health and Wellbeing privacy policy.
In accordance with the new GDPR laws please see below the information provided to you to protect your personal information.
I take your personal information and privacy very seriously and this privacy policy explains how I use any personal information I collect about you and why I collect it and I will provide every new client with the privacy policy at your first consultation.
What Information Do I Need?
I need your contact details, your: Name, Phone number, Address, Postcode, Email address and medical information in order for a treatment to go ahead, the details you provide are only known to me.
Why Do I Need It, How Will I Use It and How Long Will I Retain It?
I need personal and medical information in order to ensure that it is safe for me to provide the treatments you may ask for, also for insurance purposes and to contact you with regards to your treatment and appointment reminders. The information I receive from you is stored for my own records only and will not be shared with any third party. I will store your information for 7 years after the date of your treatment, or for 7 years after you have reached the age of 18 if you are/were under 18 when your treatment took place. Our lawful basis for processing this personal information is ‘contract’, this means that we have a legal reason to ask you for your data because we need it for contractual reasons. You do not have to share your personal data with me, but if you do not I cannot offer you a treatment at Sincerity health and wellbeing.
How I store your information.
The information you provide is stored on secure servers and paper consultation forms are locked away securely, all steps are taken very seriously in protecting your data provided and responsibly kept within the GPDR guidelines.
Marketing
I would also like to send you information about the products and services I offer using the information you have shared with me, but you do not have to agree to this for treatment to go ahead. If you agree to being contacted for marketing purposes your consent will be asked for on the day of your first treatment. You may withdraw this consent at any time by emailing me at sincerityhealthandwellbeing@virginmedia.com or calling me on 07934602079.

I will not share your information with anyone outside of SINCERITY HEALTH AND WELLBEING.

No personal information is needed for you to visit and enjoy the information on my website but if you do add information to the website you are agreeing to the terms above.
